What this policy is

DKV Mundisalud works differently from an ordinary health policy. With a conventional policy you are treated by doctors on the company’s list. That list exists here too — the whole DKV network in Spain, where care is covered at 100% — but you are not obliged to stay inside it: you can go to any doctor and any clinic in any country in the world, pay the bill and be reimbursed.

The reimbursement rate depends on the modality you choose: 80% or 90% in Spain and 90% abroad. That is why it is called a mixed policy — it combines a medical network with expense reimbursement.

This policy has no co-payments in any modality. That matters: a policy with no co-payments and full cover meets the requirements set for health insurance when applying for Spanish residency.

Five modalities

The modalities differ in their annual reimbursement limit and their coverage percentage. The higher the modality, the larger the limit and the fewer internal sub-limits.

Complet — €62,000 a year, 80% reimbursement in Spain and 90% abroad. The most economical modality, but it keeps sub-limits per visit: €40 for primary care and €80 for a specialist.

Plus — €160,000 a year, 80% and 90%. Higher sub-limits: €60 and €120 per visit.

Classic — €237,000 a year, 80% and 90%. Sub-limits per visit disappear entirely.

Élite — €310,000 a year, 90% in Spain as well. No sub-limits per visit.

Premium — €650,000 a year, 90% and 90%. Almost no sub-limits, with psychiatric hospitalisation extended to 75 days.

The hospital module can also be taken on its own: DKV Mundisalud Hospitalización Classic, without the outpatient part.

What it covers

General medicine: primary care, paediatrics, nursing and emergency care around the clock.

Specialists: cardiology, dermatology, ophthalmology, traumatology, gynaecology, oncology and all other disciplines.

Diagnostics and treatment: ultrasound, magnetic resonance imaging, CT, oncological radiotherapy, lithotripsy and PET/MR in oncology.

Hospitalisation and surgery: no limit on days, except psychiatric care, capped at 60 days a year and 75 in Premium. Single room with a bed for a companion.

Dental care: included at no extra cost, with more than 50 free procedures and the rest at special prices.

Travel assistance: for emergencies abroad, trips of up to 180 days with a limit of €30,000.

Psychotherapy: 20 sessions a year; 40 for those under 24 and in cases of eating disorders, school bullying, cyberbullying, work-related stress and domestic or gender violence.

Podiatry, physiotherapy and rehabilitation: with no limit on the number of sessions.

Pharmacy reimbursement in the individual modality. Vaccine reimbursement up to €50 per unit and €150 a year.

Annual general check-up in a single appointment: consultation and physical examination, basic blood and urine tests, electrocardiogram, vision and hearing checks.

How fast reimbursement is

Bills are reimbursed in under seven days — the shortest period on the market according to DKV. A claim can be filed from the app by simply photographing the invoice.

New in 2026

Lymphatic drainage after cancer treatment, up to 20 sessions per insured person per year.

Psychological support in neurodevelopmental disorders, for treating symptoms associated with autism spectrum disorder and attention deficit hyperactivity disorder. The number of sessions depends on the insured person’s age.

The psychology reimbursement limit rises from €40 to €75 in the Classic and Élite modalities.

Waiting periods

All benefits are available from the first day except those listed below. Hospitalisation for an emergency or an accident carries no waiting period.

Six months: outpatient surgery in groups 0, I and II; CT and magnetic resonance imaging; musculoskeletal lithotripsy; interventional diagnostics without admission; psychotherapy sessions; podiatric surgery and biomechanical gait analysis.

Eight months: surgery in groups III to VIII, hospitalisation and prostheses; assistance at any type of birth other than premature, and caesarean section; proton therapy for paediatric tumours.

Twelve months: transplants; medical care for HIV and AIDS.

Conditions of application

The policy can be taken out up to the age of 75 in the individual modality and up to 67 in the collective one. From the third year it becomes lifelong. Turning 65 does not reduce your cover: you are not moved onto a network-based policy.

The price depends on the modality, age and place of residence. Discount by number of insured: 7% for four and 12% for five or more.

I suggest Mundisalud to people whose life does not fit inside one country. Most often they are foreigners living in Spain who have kept their doctors back home, and people who travel a great deal for work. The point of the policy is a single one: you never have to check whether the doctor you want is on the company's list. Go to the one you trust, in whichever country, and get reimbursed.

It is an expensive policy, and I say so at the outset. If you live in Spain permanently, the DKV network suits you and you have no plans to be treated abroad, a network-based policy will cost noticeably less and cover the same ground. Mundisalud makes sense when that freedom to choose a doctor will genuinely be used.

What I look at when choosing a modality. The difference between Complet and Classic is not only the annual limit. Complet and Plus keep sub-limits on every visit: €40 or €60 for primary care, €80 or €120 for a specialist. Abroad, where a specialist consultation easily passes €200, that sub-limit runs out on the first visit. If the policy is being bought precisely for use abroad, you should start at Classic.

And a note on Élite: it is the first modality where reimbursement in Spain is also 90% rather than 80%. The price gap with Classic is usually smaller than people expect, and ten per cent on every Spanish bill adds up quickly. I always calculate both and show the difference in euros over a year.
