A policy for the whole team, not just the director
Integral is the most complete of DKV's business policies. It contains everything private medicine offers: GP and paediatrician, specialists without a referral, diagnostics, hospitalisation and surgery, and dental cover. The employee sees doctors through the same DKV network as an ordinary private client — the company simply pays for them at a group rate. It is taken out from three insured people. Two self-employed people without staff can join forces and take out a policy together, provided they live in the same rate zone and there are at least three insured in total. From 50 insured the policy is written as a group scheme rather than as PYMES.Two modalities: with and without a co-payment
The only difference is whether the person pays a small amount for each medical act. Integral Classic, with a co-payment. It starts at €2.30 for a GP consultation and is capped: no more than €600 a year is charged per insured person, however many visits they make. Integral Élite, with no co-payment. The premium is a few euros higher per month, and in return the employee has no reason at all to count the cost before seeing a doctor. The choice depends on why the company is doing this. If the policy is part of the benefits package and is meant to be remembered fondly, Élite is better: a co-payment, however small, spoils the impression more than the saved difference is worth. If the aim is to cover a basic need on a tight budget, Classic does the job honestly.What the company gets
Fast access to a doctor instead of a queue. The average wait in the public system is 94 days for a specialist consultation and 121 days for an operation. An employee waiting three months works worse for those three months, and then goes on sick leave anyway. A single receipt for the whole company and no surcharge for paying monthly. An allowance of €80 a day for hospitalisation from the third day, up to €2,400 a year, if the person is admitted to a centre outside the DKV network. That is money separate from treatment, and there is no charge for the benefit. Tax: why this beats a pay rise
The policy produces two distinct effects here, and it is important not to confuse them. For the company, employees' premiums are a staff welfare expense that reduces the corporate tax base in full. For the employee, insurance paid for by the employer does not count as a benefit in kind up to 500 € a year per person, or 1,500 € where the person has a disability. The limit is counted for each individual — the employee, their spouse and each child — and there is no overall family cap. Hence the simple arithmetic: a pay rise is subject to income tax and social security, and insurance within the limit is not. The company spends less and the employee receives more. Psychotherapy: 20 sessions a year. In cases of eating disorders, bullying at school, cyberbullying, workplace stress and gender or domestic violence, 40 sessions. For anyone under 24, 40 sessions in all cases. Podiatry, physiotherapy and rehabilitation with no limit on sessions. Second medical opinion in case of serious illness: internationally recognised specialists review the medical history and give their view on the diagnosis and the treatment options. Pregnancy and childbirth care, 3D/4D ultrasound and non-invasive prenatal screening by foetal DNA in high-risk pregnancies. Oncology: PET/MRI, the Endopredict genomic platform for breast cancer, oncoplastic breast reconstruction after radical surgery, and Da Vinci robotic surgery for prostate and kidney cancer. Speech therapy up to 20 sessions a year, including speech rehabilitation in adults after a stroke. Respiratory rehabilitation: 10 sessions for children under 10 and 3 for everyone else. Prevention programmes: child health, oral health, early detection of diabetes, glaucoma, breast, prostate, colorectal and skin cancer, cervical cancer prevention with HPV testing, and coronary risk assessment.What to know before taking out the policy
The premiums in the table are per insured person per month, for 2026, in groups of 3 to 15 people. The volume discount is already applied at each tier, as is the 20% campaign discount for 2026 — the figures in the table are final. Barcelona and the Balearic Islands have their own rates: they are worked out in the same system by entering the company’s address. The policy can be taken out up to age 67. Age is counted at the date the contract takes effect. Companies and self-employed people with staff evidence the headcount with the TC2 form. Self-employed people without staff need only their latest contribution receipt. Rates are reviewed once a year, at renewal.How it differs from the other PYMES policies
Integral sits in the middle of the range on price and at the top on cover. DKV PYMES Especialistas Élite costs about half as much, but has no general medicine and no hospital: only specialists, diagnostics and dental. It is chosen when the budget will not stretch to full cover for the whole team. DKV PYMES Mundisalud Classic costs about twice as much and adds what Integral does not have at all: any doctor in Spain and abroad, with 80% and 90% of costs reimbursed. Agent's opinion
With Integral I usually advise against economising on the modality. The difference between Classic and Élite is small in money and enormous in how the employee feels about it. A policy where you still pay something at the consultation is received as «the company gave us something cheap». A policy with no co-payment feels like genuine care. You spend roughly the same and get a different result in how people regard you.
The second thing I always raise at the first meeting: count your headcount and look at the thresholds of 16 and 26 insured. This is not about the discount, which is secondary. It is about who gets accepted into the policy. From 26 people no health declaration is required at all, which means an employee with diabetes or high blood pressure joins on general terms. In a small group they will either be declined or accepted with an exclusion for their own diagnosis. If you are near the threshold, it sometimes makes sense to include family members and cross it.
Third. I often hear «we would rather raise salaries». Work out both. A pay rise is taxed and attracts social security; insurance up to €500 per person is not, and for the company it reduces corporate tax in full. At the same cost to the company, the employee receives noticeably more. It is the only benefit where the arithmetic is on your side.
And an honest word about the drawbacks. The eight-month waiting period for childbirth and caesarean section is a real limitation if there are women in the team planning a child within the year. That has to be said at the outset, not when someone comes to ask. Premature birth, on the other hand, is covered from day one.
