The cheapest way to give a team private medicine
Especialistas Élite costs from €24.31 a month per person — about half the price of a full policy. The saving does not come from cutting the network or adding co-payments, but from leaving out two things: primary care and hospital. The employee has no private GP or paediatrician, no hospitalisation and no surgery. Everything else is there: cardiology, dermatology, ophthalmology, traumatology, gynaecology, oncology and the rest of the specialities; all diagnostics — ultrasound, MRI, CT, oncological radiotherapy, lithotripsy — dental cover and travel assistance. There is no co-payment on any consultation: the modality here is single, and it comes without co-payments.Who it suits
The logic is simple: in Spain you can get to a GP, even if not tomorrow, but a specialist takes an average of 94 days. Especialistas closes precisely the point where the public system fails, and does not pay for what already works. It is a workable option for a young team, for a company introducing benefits for the first time that cannot stretch to a full budget, and for the case where the policy has to reach everyone rather than just management. Thirty people at €29.75 cost less than ten at €56.93, and cover the whole team.What is missing — say it at the outset
If an employee needs an operation or an admission, those go through the public system. Under this policy a specialist will diagnose quickly, but from that point the general queue begins. It is an honest trade-off, but it has to be talked through before signing, not at the moment someone needs surgery. If the company wants to cover hospital too, that is Integral. One mitigating detail: if an employee does end up in hospital outside the DKV network, the policy pays €80 a day from the third day, up to €2,400 a year. It does not cover the treatment, but it offsets part of the cost.The larger the team, the softer the acceptance terms

Tax: why this beats a pay rise
The policy produces two distinct effects here, and it is important not to confuse them. For the company, employees' premiums are a staff welfare expense that reduces the corporate tax base in full. For the employee, insurance paid for by the employer does not count as a benefit in kind up to 500 € a year per person, or 1,500 € where the person has a disability. The limit is counted for each individual — the employee, their spouse and each child — and there is no overall family cap. Hence the simple arithmetic: a pay rise is subject to income tax and social security, and insurance within the limit is not. The company spends less and the employee receives more. Medical care for domestic, road and workplace accidents. Psychotherapy: 20 sessions a year, 40 in cases of eating disorders, bullying at school, cyberbullying, workplace stress and gender or domestic violence, and 40 for everyone under 24. From 2026, treatment of symptoms associated with Autism Spectrum Disorder and Attention Deficit Hyperactivity Disorder has been added. Podiatry, physiotherapy and rehabilitation with no limit on sessions. Second medical opinion in case of serious illness. Pregnancy care, 3D/4D ultrasound and non-invasive prenatal screening in high-risk pregnancies. Childbirth itself is not covered: the policy has no hospitalisation. Speech therapy up to 20 sessions a year. Respiratory rehabilitation: 10 sessions for children under 10 and 3 for everyone else. Prevention and early detection programmes: diabetes, glaucoma, breast, prostate, colorectal and skin cancer, HPV testing and coronary risk assessment.What to know before taking out the policy
The premiums in the table are per insured person per month, for 2026, in groups of 3 to 15 people. The volume discount is already applied at each tier, as is the 20% campaign discount for 2026 — the figures in the table are final. Barcelona and the Balearic Islands have their own rates: they are worked out in the same system by entering the company’s address. The policy can be taken out up to age 67. Age is counted at the date the contract takes effect. Companies and self-employed people with staff evidence the headcount with the TC2 form. Self-employed people without staff need only their latest contribution receipt. Rates are reviewed once a year, at renewal.How it differs from the other PYMES policies
Especialistas Élite is the most affordable policy in the range, and the only one without general medicine or hospitalisation. DKV PYMES Integral adds primary care, hospital and surgery. It costs about twice as much and covers exactly those cases where Especialistas sends the person to the public system. Agent's opinion
I propose this policy more often than you might think — and almost never as «the cheap option». It is about something else.
Look at what actually hurts in Spain. You will get to the GP at the health centre, even if not tomorrow. But an endocrinologist, a dermatologist or an orthopaedic surgeon will take three months. Especialistas removes exactly that pain and does not charge for what already works. For a company introducing benefits for the first time, it is the most honest way to start: for the same money the policy reaches all thirty people, not five managers.
But I always state the limitation out loud and ask that it be stated to the employees too. There is no hospitalisation and no surgery here. If someone finds that out at the moment they need an operation, instead of gratitude for the benefit you will get resentment. Say it in advance and there will be no complaints: people understand perfectly well that budgets are not infinite.
And one more thing. I have seen companies start with Especialistas and move to Integral a year or eighteen months later, once the business grew. That is a normal path, and within DKV it is easier than switching from another insurer: seniority is preserved. So if you are hesitating between starting small and not starting at all, start.
