A policy that does not tie you to a medical network

DKV Top Health works as a mixed system. You can be treated within the DKV network — more than 51,000 professionals and 1,000 medical centres across Spain, where care is covered directly with nothing to pay upfront. Or you can go to any doctor or clinic outside the network, settle the bill and claim reimbursement afterwards.

An important territorial condition

Free choice of doctor extends worldwide, but on one condition: your habitual place of residence must be in Spain for at least six months — 183 days — a year. If you move that residence out of Spain, cover runs to the end of the current policy year and is not renewed.

Care within the DKV network is provided in Spain.

What it covers

Practically all of medicine: specialist consultations, diagnostic tests, hospitalisation and surgery, childbirth and emergencies. Medicines are reimbursed provided they are prescribed by a doctor, bought at a pharmacy, listed in the medicines register and of therapeutic use; vaccines and allergen extracts are included too.

Dental care is covered in full apart from treatments for aesthetic reasons: endodontics, periodontics, orthodontics, fillings, dentures, apicectomies, implants and the diagnostics needed for them. Dental cleanings are covered as often as a doctor prescribes.

Three modalities

Top Health is taken out with no excess, with a €600 annual excess or with a €1,200 annual excess. The excess is the yearly amount of expenditure you bear before reimbursement starts: the higher it is, the lower the premium. One detail matters: the excess applies individually to each member of the policy, not to the family as a whole.

The maximum age at contracting is 75. From the third year of cover, protection becomes lifelong.

Who it suits and who it does not

Top Health makes sense if you move frequently between countries, are used to choosing your own doctors, or want to be able to attend a specific clinic outside Spain. If you live here permanently and the DKV network suits you, you will be paying for a freedom you will not use — in that case a network-based policy is the better choice.

How much is reimbursed outside the network

Outside the DKV network 100% of costs is reimbursed, with no expense limit. That is the main difference between Top Health and ordinary reimbursement policies, where percentages and ceilings apply.

The one exception is dentistry. Outside the network it is reimbursed partially, at 80%, and is subject to an annual limit per insured set out in your policy’s cover table. Within the DKV network dentistry involves no payment on your part: the DKV MEDICARD card is enough, anywhere in Spain.

Invoices are reimbursed in under seven days. A claim can be filed with a photo of the invoice on the website or in the app.

What the other DKV policies do not have

Assisted reproduction with only a 12-month waiting period. Other DKV policies impose 48 months, and Integral, Profesional and Modular do not cover fertility treatment at all — only a discount. Here it is covered both inside the DKV network and outside it through reimbursement of invoices: six insemination attempts, four IVF cycles with your own gametes and two IVF cycles with donated eggs or pre-embryos. The age limit is 45 for the woman and 55 for the man. The usual conditions apply: infertility in the couple, no children in common, fertile age.

Cryopreservation of semen and ovarian tissue for patients about to undergo surgery, radiotherapy or chemotherapy. No equivalent exists in the other policies.

Laser refractive surgery as cover, not a discount. Reimbursed up to a maximum per operated eye, with a 12-month waiting period. In Integral, Profesional, Modular and Mundisalud laser surgery is available only through a DKV Club voucher — a discount paid by the insured.

Reconstructive post-bariatric surgery. Over-18s, 36-month waiting period, within three years of the operation and through the DKV network. No other DKV policy includes it.

Acupuncture for pain treatment. Covered here and also in Residentes and Mundisalud. In Integral, Profesional and Modular it sits among the exclusions, alongside homeopathy and naturopathy.

Risk-reduction surgery in breast and gynaecological cancer, with a 48-month waiting period. Integral, Profesional and Modular have only the narrow version: therapeutic mastectomy of the contralateral breast for BRCA1 and BRCA2 carriers already operated for cancer.

I usually suggest Top Health to people who want practically complete freedom in choosing their medicine. The main advantage of this policy is that you can be treated in any country in the world, choose the doctor, clinic or hospital yourself, and DKV reimburses 100% of the medical expense within the conditions and limits of the policy. In other words, you are not tied to the DKV network and can carry on with the specialists you already trust. In practice almost all of medicine is covered, apart from aesthetic procedures. Dental care is provided for as well, although there a specific reimbursement limit applies. But I would not advise taking out Top Health simply because it is DKV's most complete and most expensive product. If you live permanently in Spain, the DKV network suits you and you have no need to be treated by particular doctors or in clinics outside the network and the country, you will most likely never use what Top Health offers. If, on the other hand, you travel a great deal, live between several countries, are used to choosing your own doctors, or want to be able to go to the clinic you need almost anywhere in the world — then Top Health does make sense.
