What this policy is
DKV Modular is a health policy you assemble yourself. It is divided into three modules and you take only the ones you need: one, two or all three. Those three modules produce seven possible combinations.
The fewer modules you take, the lower the premium. The doctors and clinics stay the same: you get the general DKV network — more than 51,000 professionals and 1,000 medical centres, including 93 of the 100 best hospitals in Spain. What shrinks is the extent of the cover, not its quality.
The point of the policy is to supplement public healthcare where it moves slowly. The average wait in Spain is 94 days for a specialist appointment and 121 days for planned surgery.
The three modules
Module A — primary care. General practice in the surgery and at home, paediatrics, nursing, ambulances and emergency medical care around the clock.
Module B — specialists and outpatient treatment. All medical specialities, diagnostics and outpatient treatment. Psychotherapy, physiotherapy, rehabilitation and podiatry with no limit on sessions and no waiting periods. Speech therapy, nutritionist, prevention programmes, PET/MR in oncology and one 3D/4D ultrasound a year during pregnancy.
Module C — hospitalisation and surgery. Surgery and hospital stay with no day limit, except psychiatric care, capped at 60 days a year. Prostheses and broad transplant cover. Single room with a bed for a companion. Hospitalisation allowance: €80 a day from the third day, up to €2,400 a year, provided none of the costs of the stay were borne by DKV.
Some benefits open up only in combination. The annual general check-up in a single appointment requires modules A and B together. Reimbursement for umbilical cord stem cell storage requires all three.
Included with any combination
Dental care at no extra cost: more than 50 free procedures — unlimited cleanings, consultations, extractions, oral treatments, fluoride applications and the associated dental X-rays.
Travel assistance: for emergencies abroad, trips of up to 180 days with a limit of €30,000.
Second medical opinion for serious illness: remote advice from internationally recognised specialists on the diagnosis and the treatment options.
Medical helplines: DKV doctor around the clock, paediatric, psycho-emotional, pregnancy, childhood obesity, nutrition, tropical and sports lines. Video consultations.

How it differs from DKV Profesional
The medical part of the two policies is the same: the same modules A, B and C, the same waiting periods, the same €30,000 of travel assistance and the same lifelong cover from the third year. The difference lies in the add-ons and in who each policy is meant for.
In Modular the add-ons are small and are taken out for the policyholder only: an additional capital sum for funeral expenses, a capital sum of up to €30,000 in the event of death or permanent disability, and a supplementary income for 36 months — but solely as the result of a road traffic accident.
In Profesional their place is taken by full, standalone policies: Renta, a daily benefit for any period of sick leave through illness or accident, in an amount you choose; accident cover; and funeral cover. Plus a 5% lifelong discount for the self-employed.
Hence a simple rule: if you work for yourself and need to cover the loss of income during sick leave, look at Profesional. If you only need medical care and the add-ons are of little interest, Modular.
New in 2026
Lymphatic drainage after cancer treatment, up to 20 sessions per insured person per year — module B.
Psychological support in neurodevelopmental disorders, for treating symptoms associated with autism spectrum disorder and attention deficit hyperactivity disorder. The number of sessions depends on the insured person's age — module B.
Waiting periods
All benefits are available from the first day except those listed below. Hospitalisation for an emergency or an accident carries no waiting period.
Six months: outpatient surgery in groups 0, I and II; CT and magnetic resonance imaging; musculoskeletal lithotripsy; interventional diagnostics without admission; psychotherapy sessions; podiatric surgery and biomechanical gait analysis.
Eight months: surgery in groups III to VIII, hospitalisation and prostheses; assistance at any type of birth other than premature, and caesarean section; proton therapy for paediatric tumours.
Twelve months: transplants; medical care for HIV and AIDS.
Conditions of application
The health module can be taken out up to the age of 75 and the other modules up to 67. Accident and funeral cover, from 14 to 65. From the third year the policy becomes lifelong.
The price depends on the modules chosen, age and place of residence. Discount by number of insured: 7% for four and 12% for five or more. Discount by payment frequency: 7% annual, 4.6% half-yearly, 2.8% quarterly.
Typical tasks that this policy solves
I suggest Modular to people who want private medicine but are not prepared to pay for a complete policy. You can genuinely reduce the premium here: the network of doctors is the same one used by DKV's expensive policies. What shrinks is the extent, not the quality.
The commonest mistake is to take module A alone because of the low price. Module A is general practice and paediatrics; it contains no specialists. People think they have bought health insurance and then find they cannot see a cardiologist or a gynaecologist. If the aim is to avoid the waiting lists for specialists — and it usually is — module B is essential.
The second thing I warn about: do not confuse Modular's add-ons with real insurance. The income supplement here is paid only after a road traffic accident and only to the policyholder. It is not protection against illness. If you are self-employed and need to cover the loss of income during any period of sick leave, what you need is Profesional with its full Renta. I always calculate both and show the difference.
And one last point. Modules can be added later. Many people start with A and B and take module C when the need arises or their income allows. The only thing to remember is that the waiting periods for a new module start running from the day that module is added, not from the date of the policy. So if hospitalisation is on the horizon, it is better to add C in advance rather than once the operation has already been scheduled.
